   BECOME A MENTOR!  

Mentors are asked to commit to spending one lunch a week with a student. During this time, the two can eat, talk, play, read, or just be friends. Just one hour of your week can make a difference in the life of a child!

Please print and fill out both the "Mentor Application" and the "Release Form for Background Check." If applicable, please include your email address as we prefer to send our quarterly newsletter and mentoring tips over email. Your email address will not be shared with any outside organizations.

Make sure that you print clearly and fully complete all information on the "Release Form for Background Check."

Once completed, please return both forms to the CIS Office:

Drop-Off: 6 East 4th Street, Lexington, NC (Behind the Arby's on North Main Street & beside Davidson Funeral Home)

Once your background check has been completed, you will be contacted by the CIS Office to schedule a "New Mentor Training Session."
